Locality: United States, West Virginia, Fayette, Wolf Creek - The Mouth is at the bottom of the mountain on Fayette County 82 from US 19 south of New River Gorge Bridge. Upper Wolf Creek is accessible from a roadside parking area on Fayette County 82 at the Kaymoor Mine Trailhead.

Habitat: Liriodendron tulipifera, Quercus alba, Q. rubra and Acer saccharum forest with medium to heavy leaf litter
